The following is a list of highlighted features and functionality available for the CareConnect Hub.

Automated Plan of Care

Once a plan of care is uploaded it is automatically shared with the patient's family and the system keeps track of whether the family has read and acknowledged the Plan of Care. If the plan is not acknowledged within a prescribed duration facility staff is notified by email to follow up with the family.

Lab Results Sharing

Direct connection to PointClickCare which pulls a list of lab results and presents the list to the user, either patient or relative with permission, to view in more detail. Different lab results may be visualized differently based on whether they are numeric or text based.

Facility Online Gift Shop

An online version of a gift shop with a curated list of both gift items (flowers, chocolates, cards, etc.) and services (pet visit, massage, field trip, etc..) which can be used by family members to buy things for patients. Since the family member is already logged in, and the patient is known, the buying experience is very quick with only a credit card needed to check out.

Billing

Integration with billing portion of PointClickCare for family members to review invoices and pay by credit card or ACH transfer.
